I understand that Steel City Vikings, LLC, is following best practices set forth by the CDC, WHO, and federal and state authorities for cleaning, hygiene, sanitizing, social distancing, using personal protective equipment, implementing infection control methods, as well as client and employee screening guidelines and recommendations. However, with the highly virulent nature of COVID-19, there are still contagion risks associated with in-person training. I knowingly accept these risks and agree to proceed with services at Steel City Vikings, LLC.
Our child has no physical conditions that would limit his/her participation in Steel City Vikings, LLC training activities. We hereby give permission for our child to participate in activities at Steel City Vikings, LLC, and to work on all necessary equipment. We assume all risks and hazards incidental to the conduct of this activity and transportation to and from this activity. In case of emergency, Steel City Vikings, LLC, staff has our permission to use their judgment with regards to treatment until we are contacted.
Moreover, we hereby authorize any qualified contacted physician to proceed with treatment. In case of emergency, we understand that our child will be transported to the nearest hospital by the local emergency resource if rescue squads deem it’s necessary. It is understood that in some medical situations, the staff will need to contact the emergency resources before contacting the parent or other adult acting on the player’s behalf.
Warning: catastrophic injury can result from improper conduct of this activity Further, I hereby release and agree to hold harmless and to indemnify Steel City Vikings, LLC, employees, owners, or volunteers from any claims, losses, or expenses incurred or on behalf of me, my child, or my child’s family. Note: Following an injury, a “Return to Activity From” is required prior to returning to training activities. Always notify the staff if your child is taking any medications.Participation in Vikings Hockey training programs are voluntary and can be withdrawn from at any time with 7 days notice.
All performance testing and evaluations will be performed and/or under the direction of Vikings Hockey staff.
I understand that precautions will be used during this evaluation/training program to prevent physical injury to me or my child (if a parent is agreeing to terms for a child under 18). However, in the event of physical injury resulting from the fitness evaluation procedures, equipment usage or training protocols, no medical treatment or monetary compensation will be provided by Vikings Hockey. I assume full risk associated with the participation in the training program and agree to hold harmless Vikings Hockey and employees associated with this company.
I give permission to Vikings Hockey to use my testing results in reports, publications, promotional advertising, but my identity will not be associated with such reports, publications or promotional material unless I have given specific permission to do so.
I acknowledge that Vikings Hockey is relying solely on information provided by me regarding my medical history and physical condition, in allowing me to participate in any evaluation or training session. I certify that I have made a complete disclosure of my medical history and physical condition and provided that information to Vikings Hockey staff prior to participating in any evaluation or training program. I certify that the information provided is true and correct.
Parent or Guardian Consent. If the participant is under 18 years of age. I have reviewed the information and certify it to be true and correct. I represent that we currently have medical insurance and I consent to allow my child's participation in the evaluation and/or training program.

All registered classes have a 48 hour cancellation policy. Failure to cancel within 48 hours of the start date will result in a $50 charge to cancel the class.
